Send document by email - wrong attachment

Good afternoon,
We did setup a purchase order form to be email with attachment that work well most of the time
Once in a while, the PO attached is the wrong one
The title indicates the right PO number (using {DocNo})
In the email settings, the combine page option "Sames Email address AND same DocNo" is selected
Options - Configuration indicates work and Archive file {ComAppData\...}\Work\{WinUserName}
Do you know what is causing that issue ?
